Your Initial Appointment

To see one of our doctors you will need a valid referral from your GP.

You will be asked to fill out our New Patient Registration Form prior to your appointment – this can be done online here, or alternatively you can print out the New Patient Registration Form here and bring it with you on the day of your appointment.

Please bring a copy of any investigations, reports, scans or test results concerning your condition.

You will be charged a fee to see one of our specialists, a component of which will be rebated through Medicare. Please bring your Medicare card and details of your private health insurance.

If you are being booked for a procedure, you will be provided with full informed financial consent before you proceed.

HOURS

The Specialist Centre is open 9am-5.30pm Monday through Thursday and 9am-5pm on Fridays. Our patients can access 24/7 emergency care via the Emergency Departments at St John of God Hospital Ballarat or Ballarat Base Hospital.

RECEPTION & NURSE STAFF

The Specialist Centre employs ten reception staff, lead by our practice manager Ms Kath Blackmore.

Ms Bernice Anderson, RN (Div 1) is our dedicated practice nurse. Bernie can be contacted on (03) 5331 8289 or nurse@scballarat.com.au for any patient clinical concerns.

PARKING

Parking is available directly outside the rooms on Drummond Street and we have disabled access at the rear of the rooms.
